Free Sports Physicals for CMS/HS students 7 thru 12 grade

Free sports physicals Thursday, April 21st during school.  Please stop by the office and take home your physical form to have parents sign both sides and return for Thursday. The medical history and both signatures have to be completed or the Doctors cannot see your student.
